Please view our Hen Examination Guidelines in advance of filling out the form below, so you can best describe the issue your hen is facing. If your hen is critically ill please contact your Hen Friendly Vet immediately.
This form helps us understand the issue and ensures we give you the best advice possible. If you wish, you can upload any relevant photos to support your enquiry.
Once you’ve submitted your form our dedicated team will provide tailored advice based on your description, helping you care for your hen to the best of your ability.
Please note, this form needs to include the name and postcode of the person who adopted the hen so we can match you to our records.
